TERMS & CONDITIONS
Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by Whole Body Academy, LLC during camp hours. In exchange for the acceptance of said child’s candidacy by Whole Body Academy, LLC, I assume all risks and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Whole Body Academy, LLC and all its respective representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against Whole Body Academy, LLC including all employees, all participants, or advertisers, and if applicable, owners of premises used to conduct the camp. There is a risk of being injured that is inherent in all activities.
Medical Release and Authorization
As a parent and/or guardian of the named camper, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s health if delayed.
Food
Due to the nature of the class, we ask that you were asked in the online registration form to disclose any known food allergies, sensitivities or restrictions to ensure the safety of your child. Children will be working with and in close proximity to kitchen equipment and, at times, hot surfaces. Classes may also involve handling of raw food items, including meat, fish, and poultry. Safe sanitary practices will be reviewed with the students.
Sanitation
Proper hand washing with warm water and soap is required before food preparation, after using the bathroom, sneezing, coughing or handling raw food products. Hand washing is the single most important activity that can be done to prevent food- borne illnesses
Parent Expectations
It is expected that each camper will be dropped off and picked up on time. The camp is structured according to a strict, timely schedule. Parents/guardians will be given a leeway of 10 minutes at both drop off and pick up. Habitual lateness may result in camper missing session activities, and a parental conference. If a parent/guardian is late (arriving at 4:25 pm or later) picking up their child, they will get one warning. The next time you arrive at 4:25 pm or later you will be charged $25. Please plan accordingly. *The times will change during summer camp.*
Behavioral Agreement
Campers are responsible for their actions, must respect themselves, other campers and adults, camp equipment and facilities and the environment. Campers will demonstrate communication, cooperation and caring. The following are NOT permitted by campers: leaving an activity without permission or without adult supervision, endangering the health and safety of children and staff in any way, this includes profanity, hostile language, weapons, and inappropriate behavior (ie- sexual or threatening), theft, destruction of property and all illegal substances. When the camper does not follow the behavior guidelines staff will take the following actions: redirect behavior, remove camper from activity, and possible dismissal from the camp. Dismissal/Removal from camp (camper fees) are non-refundable if a camper is sent home for disciplinary reasons.
Employee Information
Each employee has been hand selected based on an application and interview process that will best fit this camp’s mission statement. Adult employees have been given a criminal background check and have been fingerprinted.
COVID-19/FLU Information
Please note that all CDC guidelines will be in place during the duration of the program. As of now, mask wearing is optional. If anything changes you will be notified.
Hand washing will be used during the program.
Please be sure any drinking bottles that are brought in during the program have either the child’s name or initials on it. No sharing will be allowed.
**Please be mindful of traveling and who your child is spending time with outside of the program. The virus can spread easily and may not show symptoms until 2-4 days after exposure. If your child is showing any cold-like, flu or COVID-19 like symptoms, we kindly ask that you keep them home. It is imperative that we do our due diligence to keep all of our students and staff safe!
Final Agreement of Consent
I hereby give permission for my child/children to participate in all programs of Whole Body Academy, LLC. I agree that my child/children will abide by all rules and regulations adopted by Whole Body Academy, LLC. I understand that failure to comply by my child/children to these rules and regulations may result in his/her being excluded from or dismissed from participation in the program. No refunds will be given if this shall occur. I acknowledge that my child/children are both physically and mentally able to participate in all areas of the after school program. I fully understand that my child/children’s participation may entail the risk of injury. I agree to waive any claim of any kind, whether resulting in injury or otherwise, to Whole Body Academy, LLC, the facility being rented for the camp, their prospective directors, and employees from any and all liability occurring as a result of his/her participation in the program. I will be personally responsible for any financial costs incurred as a result of his/her participation in the program, including but not limited to transportation, medical expenses, and misplaced/stolen/damaged property. All sales final (health issue exception). I hereby agree to release Whole Body Academy, LLC from such liability. The undersigned has read and voluntarily signed the waiver.
